My child and I do not have the same surname - what proof of address should I provide?

You may provide proof of address in your own name, together with a document proving your relationship (for example, a family record book or birth certificate). This confirms that your child lives at the same address as you.

You must also fill in the ‘Host’ box, specifying the name of the host parent, and tick the ‘sworn statement’ box.
